- The glasswasher/dishwasher in the bar station area was leaking and overflowing. The operators mentioned that a technician was already contacted and in the middle of addressing the overflowing and leaking issue, and that some parts needed to be ordered. Please do not use the glasswasher/dishwasher until it is completely fixed and functional. In the meantime, the main dishwasher in the back kitchen must be used to wash and sanitize all glassware and utensils from the bar station.

- There was a gap on the backdoor. This would allow the potential entry of pests and vermin into the facility. The inspector informed the operator that all openings to the outside, including doors and windows, must be tight-fitting to prevent potential pest infestation.**FIX THE GAP ON THE BACKDOOR.**NOTE (2024-04-16). This violation was not looked at during the inspection; however, please be advised that this deficiency is already past its target completion date and must already be fixed by now; this violation will be checked for compliance on the next inspection.

- Several floor tiles were broken, cracked, or chipped in the bar station. Please ensure to replace all broken, cracked, or chipped floor tiles to allow the easy cleaning of the floors and to prevent or minimize the tripping hazard to employees working in the bar station.

- Please make a written procedure on how to properly and safely operate the Popcorn Machine to ensure that employees are knowledgeable on its proper and safe operational instructions. This is to prevent or minimize the risk of employees accidentally getting oil spills onto their skin when operating the machine. The written procedure is to be posted by the machine. The written procedure should also include cleaning and sanitization instructions.

- There were cleaning cloths stored on food-contact surfaces. The inspector informed the staff/operator that cleaning cloths must be fully submerged in a sanitizer bucket solution at all times after use. The cleaning cloths were stored in a sanitizer bucket solution afterwards.

- The QUAT-based sanitizer solution used to sanitize food-contact surfaces was measured at 100-ppm. The staff/manager was instructed to create a new sanitizer solution, which was measured at 200-ppm. The inspector informed the staff/manager that a 200-ppm QUAT sanitizer solution must be maintained at all times during the operations. Less than 200 ppm QUAT concentration means inadequate sanitization and more than 400 ppm means that QUAT residues could stay in food-contact surfaces, which contaminates food.

- The Food Handling Permit was not displayed. To ensure that the facility is operating with a valid permit at all times, please ensure to have a valid permit displayed in a conspicuous location in the facility easily seen by the patrons/customers.
