    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Long tangling wires were observed in Aristotle classrooms.-Please secure or neatly organize all loose wiring and keep long tangling wires out of reach to prevent tripping and entanglement hazards.
    • 06. Are proper protocols being followed in regards to client care?
      • Clean toys were observed stored in the handwashing sink adjacent to the diapering station in baby's room.-Please remove the toys and store them in a designated, clean storage area to prevent contamination. The handwashing sink must remain unobstructed and readily accessible to support proper hand hygiene practices.

      • First aid kits missing emergency blankets and tweezers.-Please provide the identified items.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Toys were not being properly sanitized. The commercial dishwasher used for cleaning toys was out of sanitizer solution.-Please provide sanitizer solution for the dishwasher and ensure that appropriate test strip is available to verify sanitizer level. Additionally, sanitizer concentration must be monitored and documented daily to ensure effective sanitation.

      • 1. Children's water bottles were observed without name tags in Aristotle classrooms.-Please label children's water bottles with their names to prevent mix-up.2. Varied concentrations of bleach solution were noted in facility's classrooms, ranging from 500 -1000 ppm.-Please dilute and maintain 1000 ppm of bleach solution concentration in all classrooms for the duration of the outbreak. Additionally, provide appropriate test strips to verify sanitizer concentrations.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • The first aid back pack in the Curies room has an epi pen and was accessible to children. Ensure this is placed up high or a locked closet to prevent potential access to the epi pen.
    • 06. Are proper protocols being followed in regards to client care?
      • There was a reusable cloth on the diapering disinfectant spray bottle. Ensure only disposable cloths are used or reusable cloths are laundered after one use.

      • 1. There was a build up of debris and dirt on several rugs throughout the facility. Ensure these are cleaned. 2. Carpets are vacuumed monthly. Ensure carpets are vacuumed daily.2. The diapering surface in the Aristotle room was taped. Replace.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Water tables are not being disinfected between use. Ensure water tables are emptied and disinfected mid day and end of day.

      • 1. Operator indicated they are not doing a soap wash of clean in place toys and wooden toys. Ensure these are washed before sanitizing weekly.2. The dress up clothes and soft toys are being laundered when dirty. Ensure these are laundered weekly.

      • There were no diapering stations/platforms in the 2 washrooms for young children.--obtain diapering stations.

      • 1) Baseboards in 12-19 month room were missing or damaged.-repair/replace/install baseboards.2) Walls in the infant-12 month room were damaged.-repair any damaged walls. All surfaces must be smooth, durable, non-absorbent and easily washable.3) Children's "Cubby" storage area were damaged and deteriorated. -repair any water damaged surfaces; all surfaces must be smooth, durable, non-absorbent and easily washable.

      • Elevated play structure in the 12-19 month room didn't have sufficient mats in the fall zone.-obtain more mats.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1) Water table frame was deteriorated with water damage.-replace/repair the water table: all surfaces must be smooth, durable, non-absorbent and easily washable.
    • 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
      • Playpens were used as cribs.-obtain proper cribs
