      • 1. Worn diapering table and shelf with exposed wood was observed in Tiny explorers' room.-Please repair or vanish the identified table and shelf to ensure proper cleaning and sanitizing.2. Torn window net was observed in 2-3 years old classroom.-Please repair or replace the net to ensure good repair.

      • Push pins and exposed staple pins were observed on some classrooms and corridor walls.-Please cover exposed staple pins with adhesive tape and replace push pins with appropriate protective materials for securing items to walls. This will help prevent potential injury and limit children’s access to hazardous materials.
    • 06. Are proper protocols being followed in regards to client care?
      • Fabric-covered diapering mats were observed in both the infant and preschool classrooms.Please replace the identified diapering mats with ones that have a smooth, non‑absorbent, and easy to clean surface.Diapering or changing mats made of foam and covered with fabric are difficult to sanitize properly and become saturated when disinfectant solutions are applied.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towels dispensers were not in good working condition. Towels were kept in open baskets in some washrooms/hand sinks.-Please repair these dispensers and ensure proper working condition. Paper towels dispensers enable clients to access the towels one piece at a time and prevent potential bulk contamination.

      • 1. Worn diapering table and shelf with exposed wood was observed in Tiny explorers' room.-Please repair or vanish the identified table and shelf to ensure proper cleaning and sanitizing.2. Torn window net was observed in 2-3 years old classroom.-Please repair or replace the net to ensure good repair.

      • Some children water bottles were noted without name tags.-Please label all children's water bottles with their names to present misuse or mix-up.

      • Medical cabinet in the gym of the daycare was unlocked and within children's reach.-Please ensure the indicated cabinet is locked at all times.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Unsteady table and display rack observed in the added classrooms upstairs.-Please fix or replace the indicated items to prevent them falling over and causing injury.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There was no soap dispenser in the newly added washrooms.-Please provide suitable soap dispensers in all washroom and classroom sinks.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kits in the daycare expansion classrooms were missing tweezers and blankets.-Please provide the indicated items.

      • Hot water temperature was measured at 20-30 degrees Celsius in most daycare classrooms.-Please adjust and maintain hot water temperature between 38-43 degrees Celsius to ensure proper handwashing process.

      • 1. Uncovered light bulbs observed in preschool classroom and washroom.-Please cover or ensure the light bulbs are scatter-proof to prevent injury in event of breakage.2. Glass jars observed in explorer's classroom.-Please remove all glass jars from reach of children to prevent injury in an event of breakage. Glass jars are only allowed under staff supervision.

      • 1. Damaged toilet floor was observed in explorer's washroom.-Please repair the indicated area to ensure proper cleaning and sanitizing of all surfaces.2. Signs of water damaged observed in observer's classroom ceiling tiles.-Please fix and ensure good repair.3. Damaged cabinet with exposed wood observed beside the diapering mat in observer's classroom.-Please repair to ensure proper cleaning and sanitizing of the indicated area.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Dirty toy bins and play equipment observed in preschool and explorer's classrooms.-Please clean and sanitize toy bins and play equipment as required.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • 1. Hot water temperature was measured at 20-30 degrees Celsius in most daycare classrooms.-Please adjust and maintain hot water temperature between 38-43 degrees Celsius to ensure proper handwashing process.2. Worn caulking was observed around preschool washroom sink.-Please apply fresh silicon sealant to the indicated area to prevent water infiltration.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • 1. Unlabeled children water bottles were observed in several classrooms.-Please label all children water bottles with their names.2. Dirty cleaning cloths being reused.-Please do not reuse dirty cleaning cloths. Cleaning cloths should be used once and then clean and sanitize or laundered to prevent cross contamination. Operator was advised to use single-use disposable paper towels.

    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There was no paper towel in the dispenser of the 4–5-year-old classroom and the preschool. Obtain paper towel for dispenser in both classrooms immediately. The handwash sink must be supplied with hot/cold running water, soap and paper towel at all times.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1.The diapering bed in the infant room was ripped on around the edges exposing the foam layer.Please replace the diaper change bed. The bed surface must be smooth, impervious to moisture and easy to clean.2. A foam was wrapped with garbage bag under the diaper change bed in the 2-3 year old room. The garbage bag was ripped in the middle exposing the foam layer. Making it uncleanable and absorbent to moisture. Please remove the garbage bag. Obtain a cover that will completely cover the foam, the cover must have a smooth surface, impervious to moisture and easy to clean. Refrain from using garbage bags because they are treated with chemicals to preserve waste which can be toxic when in contact with children.
