    • 08. Is this facility properly constructed (Non-Critical)?
      • 1) Open holes were observed in the ceiling of gym area.The light cover was not in place for one of the ceiling lights.Make sure to repair the indicated ceiling area to prevent the further damage.Replace the light covers to prevent any injury to kids.2) There was no dedicated toy washing sink in childcare, handwashing sink was also used for toy washing.Discussed to dedicate the sink for toy washing. Laundering machine can also be used for toy washing.

      • Soap and paper towel were restocked in sink in toddler room and Out of School care room.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Water temperature measured at multiple handwashing sinks accessible to kids in the facility was measured above 50 degrees C.Discussed the concerns with facility director.Make sure that water temperature is below 49 degrees C to prevent scalding.
    • 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
      • One of the crib mattresses for baby crib was not fitting properly.The mattress was removed from the crib and replaced by well-fitting mattress.

      • Windows in baby room were covered with blankets.Please ensure that appropriate curtain, blinds or covers are used for window shading.**Mattress throughout the facility are showing the signs of wear and tear, kindly repair the mattress to prevent further damage**

    • 08. Is this facility properly constructed (Non-Critical)?
      • Observed holes on the ceiling throughout the dwelling. Please ensure walls, ceiling and, floors are smooth, easy to clean, and non-absorbent.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • The indoor play area fall surfacing near the slides and climbing area is not properly secured to the floor. To prevent the surfacing from moving when a child lands and to avoid potential physical hazards, it should be properly secured to the floor.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • Water bottles were observed being stored next to hand washing stations in the room. Please ensure water bottles and sippers are not stored by the sink to prevent contamination from splashing water while washing hands. Instead, they should be kept in a safe, easily reachable location.Operator moved the water bottles to side table away from the hand sink and this infraction was fixed at the time of inspection.
