- CS20 is being reused. Test strips were not available and a reuse log was not being maintained.For each day of reuse, test strips must be used to verify that the concentration of the solution is acceptable, and a reuse log must be maintained.

- 1) Observed reusable hand towel beneath sink. Operator said it was for drying hands.-Ensure single-use towels are available for drying hands-Operator removed the towel2) Paper towel roll was on the handsink counter without a dispenser in the splash zone.-Store paper towel in an area where paper towel will not become contaminated by handwashing-Dispenser paper towel in a manner that prevents contamination of the roll-Operator sent photo of new paper towel holder that is now placed on the shelf beside the sink

- Brush for nail dust released lots of dust when brushing an object.-Ensure visible contamination is removed before treating the equipment with a product to reduce the number of organisms

- Operator did not have test strips or a log sheet, and described keeping CS20 for 14 days.-Operator must follow manufacturer's instructions when reusing disinfectant (when keeping for 14 days, maintain a log sheet and use test strips daily)-Operator elected to discard the solution daily
