- No written procedures are documented for client care in the case of accidental cuts or punctures, cleansing of the skin and mucous membrane, and dual purpose sink. Ensure written procedures are in place for cleansing of the skin and mucous membrane and client care in the case of accidental cuts or punctures. Templates have been sent via email.

- Semi-critical tools and equipment was not being adequately disinfected. A low level disinfectant was being used for disinfection. Semi-critical equipment must be cleaned and disinfected with a high level disinfectant. Ensure equipment is fully submerged in high level disinfectant and follow contact time required as per manufacturer's instructions. Ensure that test strips are available to test the high level disinfectants daily and log results.
