- The skin is not properly cleansed for an aseptic procedure. Any procedure that contacts mucous membranes or breaks the skin must cleanse the skin first then followed with an antiseptic. Ensure the skin is adequately cleansed prior to the use of an antiseptic.

- The sterility information (lot number and the expiry date) of the pre-purchased sterile equipment (permanent make up cartridges) is not being kept with that client's records. Please ensure the sterility information of the pre-purchased sterile equipment used on the client is documented with the client's forms.
