- One adult cockroach was seen on the insect sticky trap in the janitor room. The trap was dated August 1st.-Please consult with the pest control company to treat for cockroaches.

- 1. The faucet on the hand wash sink at the back entrance to the main kitchen was spraying water and not able to be used for hand washing.-Please repair.2. The taps on the following sinks were leaking:a) 4th floor kitchenette.b) 3 compartment sink in the main kitchen back area.c) 2 compartment sink by the dishwasher in the main kitchen.-Please repair the leaks.

- There was a small amount of water on the floor in and around the walk in cooler. -Please determine the source of the leak and repair.

- Main kitchen: 1. There was some food and grease build up on the floor in the area where the deep fryer was removed.-Please complete a thorough cleaning in this area to remove any grease and food debris.2. There was some mildew on the walls in one walk in cooler.-Please conduct a thorough cleaning of the walk in cooler.Kitchenettes on all floors & main dining room:1. There was some food debris build up in the cupboards.-Please thoroughly clean all cupboards in the kitchenettes throughout.

- 10. Are food temperatures during storage and processing being controlled to maximize food safety?
- Some of the small refrigerators on the care floors were not maintaining 4 degrees Celsius or less. The refrigerator on the 2nd floor was at 14 degrees Celsius. The air flow was impacted as the refrigerator was overfilled with milk, juice, and creamers.-Monitor the temperature of all refrigerators on the floors and ensure cold food is maintained at 4 degrees Celsius or less.

- Adult cockroaches were seen on a trap in the janitor room in the main kitchen. -Contact the pest control company to inspect, treat and eradicate the cockroach infestation. Provide documentation of the work completed as well as all reports provided by the pest control company. -Seal any cracks, holes, openings, and baseboards in the janitor room to prevent further entry and spread of pests.-Place more insect traps throughout the main kitchen to monitor for any activity and treat if necessary.-Increase cleaning and disinfecting of all equipment and surfaces in the main kitchen.

- 1. The main kitchen has a large area where equipment is no longer used. There was a buildup of food debris and grease in this area including on the floor, on equipment and in and on some of the cupboards, drawers, and shelves. An old gas stove had a large buildup of food debris and grease under the grates. 2. There was a build up of grease and food debris around the fryer.3. Some of the storage racks in the walk-in cooler had some food debris.4. There was some debris build up on the floor and in and on equipment in the dishwashing area. -Please thoroughly clean the above noted areas. A deep cleaning is required in order to eliminate the food source for cockroaches.

- Mouse droppings were seen in the dry food storage room in one corner and in the janitor room.-Please clean and disinfect the noted areas. Continue to monitor for any pests and consult with the pest control company.*** Update April 16th 2024 Re-inspection****1. Mice droppings were observed in a small area at the right far end of the pantry. Mice droppings was also observed in janitor room behind the area where the two storage shelves intersects. - Clean and disinfect area with mice droppings using 1000 ppm bleach. Ensure to get into the hard to reach area in the janitor room where the shelves intersects. - Keep all foods covered and continue to monitor for pest activity.

There was some debris build up behind the fridges in the 3rd floor storage room as well as in the cupboard where the toaster was stored in the 3rd floor serving kitchen and in the cupboard under the sink in the 2nd floor serving kitchen.-Please clean these areas.

- There were mouse droppings seen in the storage cabinet under the counter where the robot coupe device is stored.-Please thoroughly clean the cabinet and monitor for pests.

Organize and discard any broken or unused equipment in the main kitchen to allow for easier cleaning and pest monitoring.2. There was some debris build up behind the fridges in the 3rd floor storage room as well as in the cupboard where the toaster was stored in the 3rd floor serving kitchen and in the cupboard under the sink in the 2nd floor serving kitchen.-Please clean these areas.
