      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED DIRT/FOOD DEBRIS ON FLOOR THROUGHOUT PREP AREA. MUST DETAIL CLEAN FLOOR AND MAINTAIN THROUGHOUT PREMISES.

      • The walls and ceilings shall be in good repair and easily cleaned. OBSERVED FOOD DEBRIS ON WALL BEHIND 3-COMPARTMENT SINK ALSO OBSERVED LARGE HOLE IN CEILING ABOVE MOP SINK. MUST CLEAN AND MAINTAIN UNCLEAN WALL AND REPAIR HOLE IN CEILING.
