2304 4 St N Cranbrook BC V1C 5V9 · Residential Care - Licensing

Inspection summary

4th Street Home has been inspected 8 times by Interior Health Authority since August 25, 2015. The most recent inspection, on September 4, 2025, recorded 4 infractions.

None of the 8 inspections on record were free of infractions. No inspections are on record from the past 12 months. All recorded findings relate to upkeep, equipment or paperwork rather than direct health risks.

The number of infractions per inspection has stayed roughly the same over time.

What these findings mean

Health authorities word their findings differently, so we group them by what they mean for the people in care. Higher-risk findings are the ones that could directly make someone sick, and they matter more here because young children, older adults and people who are unwell are more vulnerable to infection.

  • Permits, records and postingLower risk

Operating permits, required signage, written procedures and logs. These findings are administrative: the documentation was missing or out of date at the time of inspection.

Recorded 13 times across 7 separate inspections, most recently on September 4, 2025. For example: 1 - Are person in care records current, complete and kept confidential?

  • Equipment and utensilsLower risk

Equipment has to be made of cleanable materials, kept in good repair and stored properly. These findings are mostly about upkeep, like a cracked surface or worn part. They are generally lower risk but show how well the place is maintained.

Recorded 3 times across 3 separate inspections, most recently on September 4, 2025. For example: 1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?

  • Food safety training and certificationLower risk

Many jurisdictions require a trained or certified person on site, such as a certified food handler. These findings mean the training record or the trained person was missing at the time. They do not by themselves mean anything was handled unsafely.

Recorded once on September 4, 2025. For example: RB1.22 - Is personal privacy respected and records and personal information kept confidential?

  • Other findingsLower risk

Findings that do not fit the categories above, such as follow-ups that confirm an earlier violation was corrected, or requirements specific to this type of facility.

Recorded 7 times across 5 separate inspections, most recently on June 2, 2023. For example: 1 - Does the facility provide food services which meet nutritional needs and preferences for persons in care?

  1. Routine Inspection

    4 infractions

    • RB1.22 - Is personal privacy respected and records and personal information kept confidential?
      • Observation(s): During the inspection of the physical facility, it was noted that one person in care's personal information was accessible on an open desk in a common area accessible by persons in care, and visitors. The binder containing personal information was relocated during the inspection to a secure locked cupboard. It was also identified that financial reconciliation records were currently stored off site at the managers personal residence. The financial reconciliation records were confirmed to have been returned to the facility on September 5th, 2025.
      • RB1.22A - Respect personal privacy and keep records and personal information confidential.
    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): During the inspection of the physical facility, it was noted that one person in care's personal information was accessible on an open desk in a common area accessible by persons in care, and visitors. The binder containing personal information was relocated during the inspection to a secure locked cupboard. It was also identified that financial reconciliation records were currently stored off site at the managers personal residence. The financial reconciliation records were confirmed to have been returned to the facility on September 5th, 2025. Failure to secure personal information poses a risk to confidentiality and privacy of persons in care. Submit by September 24, 2025, a written plan outlining how the contravention related to ensuring confidentiality of records and personal information of persons in care has been addressed. The plan must also detail the system and process that will be implemented to support the ongoing monitoring of Section 93 of the Residential Care Regulation, thereby ensuring sustained compliance moving forward.
      • R4.1V - Keep the records and personal information of persons in care confidential to the greatest extent possible while maintaining the health, safety and dignity of persons in care; 93
    • R4.6 - Are facility records current and complete?
      • Observation(s): During a review of records for persons in care, it was identified that documentation respecting money and valuables for four persons in care was not available on site. The manager confirmed that these records were currently being stored off site at her personal residence. When records respecting money and valuables are not maintained on site, there is a risk that the records may not be readily available to staff or Licensing for review. This creates potential for incomplete oversight, reduced accountability, and increased risk of loss or mismanagement of persons in care's funds and valuables. Submit by September 24, 2025, a written plan outlining how the contravention related to ensuring records for each person in care respecting money and valuables has been addressed. The plan must also detail the system and process that will be implemented to support the ongoing monitoring of Section 91(2)(a) of the Residential Care Regulation, thereby ensuring sustained compliance moving forward.

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): During a review of the physical facility, it was noted that areas of the environment require repair and maintenance. Observations included chipping paint, scratched and chipped door frames, and a hole in the drywall within one bedroom. In addition, the driveway was observed to have uneven concrete, which poses a potential tripping hazard. Failure to maintain all rooms and common areas in a good state of repair poses potential risk of unsafe conditions and does not support the health, safety and dignity of persons in care. Submit by September 24, 2025, a written plan outlining how the contravention related to ensuring all rooms and common areas are in a good state of repair has been addressed. The plan must also detail the system and process that will be implemented to support the ongoing monitoring of Section 22(1)(b) of the Residential Care Regulation, thereby ensuring sustained compliance moving forward.
