- Baby Room: Cleaning chemicals were observed stored in an unlocked cabinet under the sink. Staff were advised to secure and lock the cabinet to ensure chemicals are inaccessible to children and to prevent any potential chemical hazards. The drawers were subsequently locked by staff upon request.

- Laundry Room: Children were observed passing by the laundry room, which was completely open while a mechanical dryer was in operation. This presents a significant safety hazard. The operator was instructed to keep the laundry room door closed at all times, as children may be exposed to risks from operating machinery and cleaning chemicals.Similarly, the mechanical room must remain secured at all times. Access must be restricted to authorized staff only and strictly prohibited for children to prevent potential injury or exposure to hazards.

- Kinder Room: An electrical switch in the washroom was observed to be loose and detached during the inspection.The operator was instructed to have the switch repaired immediately. In the interim, staff were directed to use an alternate washroom to ensure children are not exposed to the risk of electrical injury.

- Baby Room: The diapering pad was found to be ripped, compromising its ability to be properly sanitized. The operator was instructed to replace it with a new one.

- Toddler Room: Diapering procedures were observed to be performed incorrectly by staff. Staff did not wash their hands between children, and no disinfection was conducted after diaper changes. This is a critical violation of infection control practices. PHI intervened immediately and provided education to staff, emphasizing strict adherence to the diapering procedures posted on the wall. It was clearly communicated that changing gloves is not a substitute for proper hand washing.Hand hygiene is mandatory after each diaper change to prevent the spread of fecal-oral diseases and potential outbreaks. Additionally, proper disinfection of the diapering area after each use is essential and must be consistently performed.

- Staff Washroom: No paper towels observed in staff washroom hand washing sink. Operator was advised to stock the paper towels to ensure sanitary hand drying options.

- Facility: The facility’s hot water temperature was measured at 63°C throughout. The operator was advised to reduce the temperature, as excessively hot water can cause burns and pose a risk to children. Operator change the setting from knob to bring down the temperature during the inspection.

- Caulking around all toilets in the toddler room Adult and Children washrooms needs to be redone to ensure floors are smooth, impervious to moisture and easy to clean. Caulking must also be redone for all floor/wall seams in these bathrooms.

- There is masking tape residue on the floor of the preschool room that must be removed to allow for proper cleaning and disinfection of floors.

- Preschool B's sanitizing solution was tested and was found to be approximately 10 ppm. Was remade by operator.

- The furthest left tap fixture in the toddler room washroom was loose. Operator tightened it during inspection.

- Bleach solutions were tested in multiple rooms and were found to be 0 ppm chlorine. At the request of PHI, all bleach solutions were remade and were verified to be 100 ppm.

- The facility was using lemon-scented bleach at time of inspection, which is not suitable for disinfection of surfaces. Unscented bleach was available and the lemon-scented was discarded.
