      • 1. Pre-Kinder classroom: The electrical cord from the television is accessible to children. Please ensure all electrical cords are secured and inaccessible to children.2. Pre-Kinder classroom: Pushpins are being used on the wall. Please remove them, as they may fall onto the floor and cause injury to children.3. Room 3: The tall white shelving unit, approximately 5 feet in height, is not anchored to the wall and is easily movable. Please securely anchor the unit to the wall.

      • 1. Toddler room: Printing paper used for crafts was observed stored underneath the diapering mat. Staff were instructed to discard the papers. Please ensure the diapering surface is used only for diapering purposes.

The alternative restraint available to prevent a child from falling is not suitable, as it consists of Velcro and elastic components. Please securely anchor an appropriate safety belt to the change table.2. Room 2: The safety belt attached to the diapering table is not made of suitable material, as it consists of elastic and Velcro components. Please replace it with an appropriate safety restraint made of durable, cleanable material.

      • 1. Pre-Kinder classroom: The alcohol wipes in the first aid kit are expired, and supplies such as an emergency blanket and tweezers are missing. Please ensure the first aid kit contains all required supplies and that all items are within their expiry dates.

      • 1. Toddler room: One edge of the sink counter is in disrepair. Please repair to ensure the surface is smooth, non-porous, and easily cleanable.2. Pre-Kinder classroom: Several sleeping mats are ripped and have been repaired with tape. Please replace the affected mats to ensure they are smooth, non-porous, and easily cleanable.3. Toddler classroom: Several sleeping mats are ripped and have been repaired with tape. Please replace the affected mats to ensure they are smooth, non-porous, and easily cleanable.4. Pre-Kinder classroom: The fabric storage baskets on the bookshelf are dirty.       • 1. Toddler room: Cleaning rags were observed hanging on the faucet. Please ensure the following:Cleaning rags are used only once and then disposed of or laundered. This prevents cross-contamination and bacterial spread.Clearly labeled bins are provided in each classroom for the collection of used rags.2. Toddler room: A bleach solution at 1000 ppm is being used to sanitize tables and toys without rinsing. Please ensure tables and other applicable surfaces are rinsed with water after sanitizing with a 1000 ppm bleach solution.

      • 1) A blanket was hanging from the diapering table.2) A milk tray and a bag containing children’s bedding were observed on top of the diapering mattress at the time of inspection.*Staff placed the blanket and bedding in the laundry bin.The milk tray was sent to the kitchen for proper washing and sanitizing.**The diapering table and mattress must be used solely for diapering purposes. The diapering surface must not be used for storage of items such as bedding, trays, or other materials.

      • 1. There are various foam tiles in room 1 (infants) that are peeling. 2. The paint on the walls are peeling in the washroom of rooms 1 and 4.1. and 2. All surfaces must be smooth, easy to clean, and in good repair. Please replace and repair these areas.

      • There are expire alcohol/antiseptic wipes in the room 4 first aid kit.Replace and replenish the wipes in all first aid kits as needed.

      • There are various areas around the daycare where the paint is starting to chip/peel off of the walls.Please repair and repaint these areas to prevent the accumulation of debris and to facilitate proper cleaning of the walls. Repairing the walls will prevent children from peeling or picking at the walls.

      • The bottles of bleach sanitizer in each of the classrooms are not labelled properly.Please label the bottles with the appropriate chemicals or contents in the bottle.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. There is a thumb tac on the bulletin board in Room 3.2. There is no electrical cover on the electrical outlet in Room 3 on the left hand side of the room.1. and 2. Both concerns were addressed immediately by the operator.

Replace the diapering change mat so that it is in good condition and easy to clean.2. Replace or reupholster the sleeping mats so that they are in good condition and are easy to clean.

      • 1. There are various areas throughout the classrooms (room 1, 2, and 3) where the wall is damaged and paint is peeling off the walls.2. The ledge near the window in rooms 2 and 3 are damaged.3. The caulking around the hand wash sink and counters in the classrooms are starting to discolor and have a build up of debris.1. and 2. Refinish and repaint the walls so that they are in good condition and are easy to clean.3. Replace the caulking around the sink and counter areas.
