    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • The most recent client care record does not have any sterility assurance documentation kept with the record. With the infraction of disorganization and missing sterility info, this increases the risk of non-sterile equipment being used on clients.REQUIREMENT: Effective immediately, client care records must have sterility assurance documentation from all items classified as 'critical' kept with the record.

    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • A visibly soiled waxing strip was kept in with clean strips at the very top.This strip was discarded during the inspection; the other strips did not appear visibly contaminated.

      • Beds used for instructional purposes the prior day were not reprocessed, and bed sheets were not laundered (visible waxy stains were on the sheet).REQUIREMENT: Beds/tables/etc. which is used by clients for a personal service must be properly cleaned and disinfected. with all linens being reprocessed and no visible staining noted.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • 1) Disassembled tattooing machine handpiece had minor amounts of debris inside the handpiece.2) A couple tools inside the mani/pedi packaging had debris inside the cuticle clipper.REQUIREMENT: All reusable tools must be fully disassembled, cleaned, and disinfected, all while visual checks for cleanliness are conducted.
    • 20. Is reusable equipment disinfected properly?
      • 1) Expired CS20 was used on tools (Expired since Feb 2024)REQUIREMENT: Obtain non-expired CS20. Do not use expired disinfectants to reprocess tools.2) Expired test strips (2017 and 2022) were available, with 2022 located still inside the box unopened. Instructors admitted to reusing the same poured disinfectant over 3-4 days without any test strips being used.REQUIREMENT: Disinfectants which have the capacity to be reused over days or weeks must be tested once a day to verify the concentration is still sufficient.3) No disinfection log was being maintained by the Instructors/Operators.REQUIREMENT: Disinfectants which are poured and reused over days/weeks must be tested and the results of the test be maintained in a disinfection log.4) Only 1 container of Preempt RTU was not expired in a facility with 4 3-4 service rooms and 1 instructional room.REQUIREMENT: Obtain more disinfecting wipes to ensure client contact and reusable equipment is being properly cleaned and disinfected.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Both sterile and non-sterile tattooing/PMU equipment are kept in the same drawer with no organization. Director stated some of this equipment was to be used for practice, but there is no indication of which equipment that would be since there's no label and sterile/non-sterile equipment is fully mixed together.REQUIREMENT: Immediately separate sterile and non-sterile products from each other and do not store together. Ensure all sterile products are stored using a first-in-first-out system. to ensure proper inventory of sterile supplies is maintained.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • User manuals for energy emitting devices (e.g. ILP Laser) were not readily available.REQUIREMENT: Obtain and ensure that a user manual for all energy-emitting devices is available when needed.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • 1) Numerous amounts of expired disinfectant kept onsite.REQUIREMENT: Discard all expired disinfectants according to what the manufacturer specifies to ensure these expired products are not used by staff and students.2) Spray bottles containing floor wax cleaner and skin wax cleaner were not properly labelled.REQUIREMENT: All spray bottles must be properly labelled to ensure there is no confusion on the intended use of the product.
