    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - Duct tape was used to secure carpet in place in Classroom 4. The duct tape was peeling and no longer effectively affixing the carpet, creating a potential tripping hazard.*Remove the peeling duct tape and repair or secure the carpet using a durable, appropriate method to ensure the flooring surface remains safe and in good repair.

      • - Tables and chairs need to be refinished or repainted to facilitate proper cleaning and disinfecting.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • - Bleach sanitizer concentration at the diapering station tested at 50 ppm chlorine. Sanitizer used for toys and tables tested at 1000 ppm chlorine.*Staff reprepared the bleach solution and it was tested as 100 ppm.
