      • -An antiseptic product was not being applied prior to a service that will pierce the skin. Ensure an antiseptic product is used.

      • Semi-critical tools were disinfected using Pre-empt RTU (an intermediate-level disinfectant).The operator was advised to use Pre-empt CS20 available in the facility to disinfect the semi-critical tools.

      • -High-level disinfectants are not being tested and verified daily using proper test strips. The test strips were expired.-High-level disinfectant log was not maintained.Ensure that the high-level disinfectant (HLD) is tested daily using the test strips specified by the disinfectant manufacturer and HLD log is maintained.
