    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for preparation of skin were completed incorrectly - cleansing of skin and antiseptic application.Written procedure for post service care did not include all services that would require post service care.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • Unsuitable sterilizer located in the treatment room - sterilizer does not utilize steam.Discussed issue with operator and she indicated that she would only use the equipment for her own personal use. Remove this equipment from the personal services facility.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • Several devices that emit energy were identified during the inspection and manufacture instructions available were incomplete and/or not readily available. Provide complete manufacture instruction manuals to EPH for review for all energy emitting equipment and keep copies onsite readily available for review.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Electrolysis machines are class II medical devices; therefore, the manufacturer of this device must hold an MDL - Medical Device License. Proof of this license must be provided to EPH prior to using this device in a personal services facility.If proof cannot be provided, then the device must be removed from the facility.
