      • * Chlorine test papers were not available onsite. Bleach water solution is used for toys washing. Please ensure chlorine test papers are available onsite for measuring concentration of the bleach water solution.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • *Wood playhouse surface was unfinished/had sharp surfaces. Please ensure playhouse is properly finished, surface is durable, and easy to clean.

      • Floor heater cover close to the incliner chair in the bunny room was broken.-Please replace/repair cover
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Cleaning is required in the following areas1. Hard to reach areas behind the couch and toy house on the ground floor- dirt and debris built up.2. Underneath the incliner chair in the bunny room - litters from the bunny cage Please ensure hard to reach areas are cleaned regularly and general cleaning of the bunny room.

      • 1. Wet sand was found in the sand play table in the outdoor play .Please discard wet sand to prevent microorganisms growth and contamination.2. The fence in the outdoor play area with elevated ground was at below 6ft.Please elevate fence to be up to 6ft. Addition of wooden fence is recommended.

      • Cleaning is required birch toys shelf by the farmers market sign - dust and food debris built up. Please make sure containers and toys are cleaned as well.

    • 07. Is this facility properly constructed (Critical)?
      • *Gate needs lock at the deck area. It must be secured.*NE corner of the deck - Fall zone heigh above the ground 50inches. Guard height was 42 inches. Operator will secure this area with storage self to close this area access to kids. Kids will be with adult outside at the deck. It is not a playground space.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • * Wooden stumps -gaps on the surfaces are filled or they are removed from the center*Lego table-little part of it was not finished- Please install the rest of the missing lego table surface.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Potability of the water source was unknown. Town is doing maintenance of the water cistern and water testing monthly. Water testing will be done before start to operate and satisfactory result is required.
