    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • 1. The paper used for client surfaces in the waxing room was clearly used and not disposed of after client use. Operator is advised to dispose of the paper on client surfaces as soon as service to client has been completed.2. Porous eyelash/eyebrow brushes observed to be reused without cleaning, as the buildup of organic materials was visible. Operator is advised to not reuse porous brushes and to clean all tools with soap, water and disinfectant if intended for reuse.3.Used single use items, including nail files were being stored at the workstations.Please ensure that single use items are be disposed of after use.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Dirty ready-to-use nail clipper and scissors were observed at time of inspection. Operator is advised to visually confirm all re-processed tools are free of any visible debris prior to submerging them in the CS-20.
    • 18. Is reusable equipment cleaned properly (Non-Critical)?
      • 1. Containers storing cleaned tools and equipment have a visible accumulation of debris. Please ensure all equipment is stored in a clean container or clean environment. 2. Nail soaking tubs that are stored and ready to use show an accumulation of visible debris. Please ensure all equipment is cleaned thoroughly after each client.3. The waxing station in the waxing room is not maintained in an adequately cleaned state. Please ensure all equipment is maintained in a clean and sanitary state. 4. The eyelash and eyebrow tinting jar is visibly discolored and not maintained in an adequately cleaned state.Please ensure all equipment is maintained in a clean and sanitary state. 5. Tweezers packaged in the waxing room have multiple paint chips and are no longer smooth and easily cleanable.Please do not use on clients any tool that has become no longer easily cleanable.
    • 20. Is reusable equipment disinfected properly?
      • Records of daily testing were not being taken. Please ensure that the CS-20 is tested everyday it is used, that the result of the test is recorded and that the solution is discarded after 14 days of disbursement or if the test fails before the 14 day life span is up.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • The container used for the scoop for the paraffin wax was sitting in the paraffin wax container.Please place scoop outside of paraffin wax container to protect the bulk supply from contamination.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Client chairs for the pedicures are not maintained in a state of good repair, rendering them non-smooth, porous and not easily cleanable. Please ensure all damaged chairs are resurfaced with material that is either laundered frequently or repair the damaged areas.

The CS-20 was expired as of May 2023. Please ensure all disinfection solutions are not expired when in use. 3. Records of daily testing were not being taken. Please ensure that the CS-20 is tested everyday it is used, that the result of the test is recorded and that the solution is discarded after 14 days of disbursement or if the test fails before the 14 day life span is up.

    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • -The operator has not notified EPH of the business and activities that the business currently offers. Please submit all applicable information to the RHA as soon as possible
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • -Written procedures template was filled out at the time of inspection. Ensure that it is shared with staff and stored in an easily accessible location
    • 20. Is reusable equipment disinfected properly?
      • -Pre-empt CS20 expired. Please purchase a new disinfectant.-Operator indicated nail drill tops are soaked in alcohol. Please switch to Pre-empt RTU as discussed.-Foot files were not being soaked in disinfectant. Please ensure that they are fully immersed in a high level disinfectant.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • -Scoops were stored in the foot scrub. They were removed at the time of inspection.
