    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures are in place. Proper instructions were provided how to complete the written procedures.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The client bed was observed with exposed threads and damaged joints, including torn leather, which compromised the integrity of the surface. These conditions prevent the bed from being smooth, non‑porous, and easily cleanable. The operator was advised to either repair or replace the client bed to ensure it is maintained in a sanitary condition and suitable for client use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • A reflective mirror was observed in the room where a laser machine is installed. The operator was educated on the potential safety risks associated with reflective surfaces in laser treatment areas, as mirrors can reflect laser beams when equipment is operating and cause unintended eye or skin injury to both staff and the public. Upon request, the operator removed the reflective mirror from the room.
