      • Corrective Action(s): Have the motor repaired to prevent any further leaking of what appears to be oil.
      • Also have the syrup nozzles repaired or replaced so they are no longer dripping as this could be an attractant to pests.

      • Observation (CORRECTED DURING INSPECTION): Found chicken cooling in walk in cooler not protected from contamination
      • Corrective Action(s): Ensure all food is covered with either a lid, or plastic/metal wrap. If cooling, keep corner of covering open to allow for heat to escape.

      • Observation: 1. Mice droppings noted throughout premises on shelves, inside cupboards, and on the floor under equipment and shelving units.
      • 2. Fruit flies noted in the kitchen and the bar area.
      • Corrective Action(s): Pest control is on contract, traps noted through out the premises. Traps will not work with out also removing food and water sources.
      • 1. Complete extensive cleaning of entire premises to remove all food, grease and water build up including under hand sink in servery, under pop syrup boxes, and pizza prep area.
      • 2. Disinfect all areas where droppings noted to prevent spread of disease and prevent further attracting pests.
      • 3. Back door by the pop syrups has a hole to the outside, new weather stripping may be needed.
      • 4. Continue to work with Pest Control to eliminate problem.

      • Observation: 1. Found food debris in hard to reach areas under shelving, and equipment, between equipment, especially in the pizza prep area, pop syrup boxes and under sink in the servery section
      • 2. Found mouse dropping on shelving units, and cupboards through out premises
      • 3. Found have food build up inside pan storage bins.
      • 4. Cooler door handles had a build up of debris under the handles.
      • Corrective Action(s): A deep and thorough cleaning of all floors, shelves (units and cupbaords), handles and inside coolers/freezers.

      • Observation: Food inside the Pizza prep cooler and the middle front cooler were found to be 7 C.
      • Corrective Action(s): All cooler units must be able to maintain 4 C or colder at all times including busiest times of the day.
      • Have units repaired and or replaced so they can maintain 4 C or colder.
      • Only keep small portions of food that can be used in 2 hours or less in the cooler until repaired.

      • Observation: Glasswasher is not dispensing any sanitizier. Iodine level tested at 0ppm. For effective sanitation, iodine levels must be >12.5ppm.
      • Corrective Action(s): Operator is to immediately stop using the glass washer and fix the glass washer to ensure sanitation level are met. In the meantime, sanitize all glassware in the low temp dishwasher in the kitchen.
