    • 08. Is this facility properly constructed (Non-Critical)?
      • The flooring outside the bathroom downstairs was ripped and in disrepair. **Repair the flooring to prevent injuries and to ensure the surface is easy to clean.**
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • In the infant room, a belt buckle for a high chair had food caked on and was visibly dirty. **Clean or replace the belt buckle. Ensure the chairs and buckles are cleaned and sanitized after meals.**
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. A plush toy with a battery compartment was found in the older toddler room. These types of toys cannot be properly cleaned and disinfected. **This toy was removed and set aside.**2. In the infant room, towels used to wipe water off the floor from the water table was stored on top of the water table.**Towel was removed. Do not store floor towels on top of tables, chairs, toys, and other surfaces kids comes in contact with.**

      • A few rooms had used staples without tape over the staples.**Please go through all the rooms and ensure tape is covering all the staples.**
    • 10. Is a properly supplied first aid kit provided and readily available?
      • A few of the first aid kits were missing alcohol wipes.**Replenish the first aid kits with alcohol wipes.**
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • A few rooms were storing cushions/toys/crafts on top of a child's cot alongside their bedding.**Please do not store anything on top of a designated child's cot and on their bedding.**

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • The laundry room door was left open making it accessible to children. Cleaning and disinfecting chemicals were stored in the room,- The door was closed during inspection. Ensure to keep the laundry room door closed at all times to prevent children's access.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • A bag of pit balls and scarfs used for dance were stored in the children washroom located in the teddy bear classroom.- The Scarf and bag of balls were removed from the washroom during inspection. Ensure that these items are cleaned and disinfected before using them.

      • Two chairs in the Jellybean classroom were observed to be torn in many areas. -Obtain a slipcover or resurface the chairs in disrepair.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Rooms 1 and 2 push pins were observed in bulletin boards. Remove pins and replace with tape, or staples covered with tape. 2. Room 3: Shaving cream, shampoo and conditioner were stored in reach of children. Store potentially harmful products away from children. The use of these products must be closely monitored. Corrected during inspection. 3. Play dough made on site is made with raw flour. Ensure that flour is cooked to 74C before use.
    • 06. Are proper protocols being followed in regards to client care?
      • Rooms 1 and 3 diapering tables observed to have a build-up of debris under the changing mat. Ensure to clean and disinfect on top and below the changing mat.
    • 15. Is laundry being properly handled in a suitable manner?
      • Wash cloths used for cleaning and disinfecting surfaces are reused throughout the day. Wash cloths should be changed at every use. Or paper towel can be used in its place.

      • 1. Vinyl lined chairs and blocks throughout facility are showing signs of disrepair - Cracks in fabric, ripped seams. Repair chairs so that they are smooth and impervious to moisture. *Prioritize items in rooms 1 and 3*2. Room 3: Paint chipping on ledge near exit door. Carefully refinish the paint in this area so that it is smooth, non-porous, and easily cleanable. 3. Wicker baskets were used for toy storage throughout facility. Replace wicker baskets with plastic baskets that are smooth, non-porous and easily cleanable.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Bags covering napping mats does not sufficiently cover the entirety of the bedding. Ensure that client linens to not come in contact with each other at any time.
