   • 06. Are proper protocols being followed in regards to client care?
     • 1. During observation of the diapering process in the Explorers room, the staff member did not change gloves after removing the soiled diaper. The same pair of gloves was used to redress the child, assist with handwashing, handle the sanitizer bottle, and operate the paper towel dispenser.Education was provided on site. Please ensure that contaminated gloves are removed and replaced prior to touching clean surfaces, equipment, or supplies.
