      • 1. Sanitization logs were incomplete in several classrooms, including Infant 5, Toddler 1, Toddler 2, and Kindergarten.Staff reported that cleaning is done daily but logs are not being consistently filled out.Please ensure logs are completed daily to maintain accurate records.Nov 5, 2025: The cleaning checklist was not completed for Toddler 2 room for November 3, 4 and 5.

      • 1. Kindergarten Room: The bleach sanitizer was dark in color, and black residue (potentially mold) was noted inside upon opening.Please discard the contaminated bottle and replace it with a new one containing fresh sanitizer.2. Infant 4: One of the childproof locks were removed for one of the cabinets. A new lock was installed for the cabinet during the inspection.

      • 1.Toddler 2 room: During a diapering procedure for a child who had a fecal accident, the staff member sanitized the diapering mat but did not sanitize the safety straps.Education was provided on site. Staff were reminded to clean and sanitize all surfaces, including diapering straps, to prevent the spread of disease. The staff member wiped down the straps after the discussion.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1. Preschool 3 and Preschool 4: The paper towel dispensers at the staff handwashing sinks were found to be empty during the inspection.Staff replenished the paper towels during the inspection.

Sanitization logs were incomplete in several classrooms, including Infant 5, Toddler 1, Toddler 2, and Kindergarten.Staff reported that cleaning is done daily but logs are not being consistently filled out.Please ensure logs are completed daily to maintain accurate records.1. The washing machine is not being maintained in a clean condition. Hair and debris were observed on the exterior and inside the rubber gasket.Please clean and sanitize the machine thoroughly to ensure proper hygiene.
