- 1) Flooring in the bakery area is in disrepair. A few tiles missing or damaged. 2) The counter/cutting board where slicer is stored in the meat department had deep grooves and no longer smooth and easily cleanable. Requirements:1-2) Please repair/replace. All surfaces must be smooth, easy to clean and non-absorbent.

- 1) Paper towel dispenser at the deli hand sink was empty at the time of inspection. Paper towel roll was stored on the counter. - Corrected during inspection. 2) No paper towel dispenser available at the cheese island handwashing sink. - Please install a paper towel dispenser at or near the hand sink in this area.

- Gap observed alongside the bottom of the exterior doors at produce receiving and grocery warehouse areas. Please install weatherstripping to prevent the entry of pests into the facility.

- 1) Cleaning required under the shelves and sink in the produce preparation area. 2) A buildup of dust observed on the fan covers in the meat department. - Please clean and maintain clean.

- Concentration of freshly made Quat sanitizer from the dispenser was measured greater than 400 ppm. Quat sanitizer must be used at 200 ppm.
