     • Broken wall tile behind pizza cutting table.Holes in wall / unfinished surfaces (near ceiling) on the outside of the walk-in cooler.Lower shelving in front area is chipped / damaged and no longer smooth and easily cleanable.ALL SURFACES must be maintained in good repair.Fix / repair as necessary.**Outstanding violation - this needs to be corrected**
   • 23. Is the facility maintained in a clean and sanitary condition?
     • The floor under the shelves in the walk-in cooler was dirty with food/debris, etc.-clean the floor, wall, shelves, etc in the walk-in cooler. Move shelves to thoroughly clean. This must be cleaned on a regular basis.
