      • Small plastic pieces were found on the windowsill in the infant room, where they were accessible to children.The operator was instructed to immediately remove the plastic pieces to eliminate choking hazards.

      • Child safety locks were missing from the handwash sink cabinets located in the infant and toddler rooms.Please install child safety locks to prevent access to hazards.

      • 1) The large rug located in the infant room was soiled in food stains and particles.Please deep clean the rug.2) Water stains were found on the ceiling tiles throughout the facility.The operator informed the inspector that the leaks have been addressed. Please replace the water damaged ceiling tiles.

      • Hot water was unavailable from the tap located in the Pre-K washroom.Please repair the handwash sink to ensure hot and cold water are supplied to the taps.

      • Bins of supplies, toys, and miscellaneous items were stored in the washroom located in the toddler room and accessible to children.Please store items in a designated storage area that is inaccessible to children to eliminate hazards and prevent cross-contamination.

      • Mold and mildew were observed around the rubber gasket of the laundry machine.Please clean and disinfect the indicated area to ensure all the mold and mildew are removed.

      • There are thumbtacks on the bulletin board in the toddler room (in the reading corner).Please remove thumbtacks and find another alternative to hang posters and sheets as the thumbtacks can fall off the board and become a sharps and choking hazard.

    • 06. Are proper protocols being followed in regards to client care?
      • There was a bed linen covering the diaper change bed in the infant room. upon discussion with the staff. it was discovered that the bed linen is changed at the end of the day. The diaper change bed was also ripped exposing the foam layer.Please remove the bed linen. Obtain a single-use liner that must be changed between each use. Discard and replace the diaper change bed.
