      • Observation (CORRECTED DURING INSPECTION): Milk cartons were stored above 4C in the front stand up cooler. Staff confirmed that milk was delivered less than 30min ago. All milk cartons were moved to walk in cooler.
      • Corrective Action(s): Please ensure that cold potentially hazardous food is stored at or above 4C.

      • Observation (CORRECTED DURING INSPECTION):
      • REPEATED VIOLATION
      • 1. Low temp mechanical dishwasher is used for warewashing. Chlorine residual recorded at 0ppm after multiple runs. This could result in inadequate cleaning and sanitizing of dishes. Staff to use 2 compartment sink for manual dishwashing. Drain plugs and QUATS sanitizer dispenser available.
      • 2. QUATS sanitizer in spray bottle was recorded at high concentration >400ppm which is above the acceptable range. This could result in potential food contamination when used for sanitizing food contact surfaces. Staff made new sanitizer solution and concentration recorded between 200-400ppm.
      • Corrective Action(s): Please ensure that all food contact surfaces/dishes/equipment are adequately cleaned and sanitized.

      • Observation: Pink mildew/mouldy spots observed on the inner curtains of ice machine. This could result in direct contamination of ice/food.

      • Observation: In the stand up freezer, raw chicken was observed in the garbage bag. Raw chicken was in direct contact with the bag. Please note that the garbage bag is not made of food grade material and can seep harmful chemicals/pesticides into the food. Staff discarded around 2 pounds of chicken at the time of inspection.
      • Corrective Action(s): Please ensure that an acceptable food grade material is used for storing food.

      • Observation (CORRECTED DURING INSPECTION): - Low temp mechanical dishwasher was primarily used for dishwashing. The concentration of chlorine residual recorded at 0ppm at the time of inspection. This could result in inadequate cleaning and sanitation of food contact surfaces.
      • - 3 compartment dishwashing sink to be used for manual dishwashing temporarily. 200ppm QUATS recorded at QUATS dispenser mounted above dishwashing sink. Drain plugs onsite.
      • Corrective Action(s): - Do not use dishwasher until it has been repaired.
      • - Ensure that all utensils/food contact surfaces are properly washed and sanitized.

      • Observation (CORRECTED DURING INSPECTION): Handwash sink in the middle area of the kitchen lacked paper towels. Staff refilled the paper towel dispenser at the time of inspection.
      • Corrective Action(s): Please ensure that all handwash sink have adequate supply of hot and cold running water, liquid soap and single use paper towels during facility operation.

      • Observation: 1. Pink mildew/mouldy like spots observed on the left corner and white tray inside the ice machine
      • 2. 4x cutting boards had knife cuts on the surface (both sides) and black stains.
      • 3. 1 wooden spoon was half broken and was used for handling food. Staff discarded the broken spoon at the time of inspection.
      • 4. Small bowl was observed in the small salt bulk bin. Staff took it out of the bin during inspection.
      • Corrective Action(s): Please follow proper food handling, storage and safety practices to prevent the risk of food contamination:
      • 1. Clean and sanitize the ice machine to prevent the risk of contamination of ice stored in it.
      • 2. Discard the above mentioned cutting boards and replace them with new ones as the cuts/crevices in the boards can not be adequately cleaned and sanitized and over time harbour harmful pathogens which may result in food contamination during food prep.
      • 3. Always use scoop with handle for dispensing food out of bins.

      • Observation (CORRECTED DURING INSPECTION):
      • 1. Dishes are not being sanitized in the low temperature dishwasher as even after multiple attempts, there was 0 ppm chlorine residual.
      • 2. It was observed that one staff member washed and rinsed two pasta pans, without sanitizing and then dried using a cloth sitting at the 3-compartment sink. The staff member then proceeded to prepare more pasta in these same pans.
      • Corrective Action(s):
      • 1. Use the 3-compartment sink for manual dishwashing to wash, rinse, sanitize and then air dry all dishes. As it is unknown since when the dishwasher has not been sanitizing dishes, ensure to wash, rinse, sanitize and air dry any dish/equipment before being used.
      • 2. Review proper dishwashing methods with all staff. Ensure dishes that are manually washed are washed with liquid soap, rinsed with clean water, sanitized with an approved sanitizer (QUATS or chlorine at a concentration of 200 ppm) for a minimum of 2 minutes before being air dried. Do not towel dry.

      • Observation (CORRECTED DURING INSPECTION):
      • 1. No staff observed to wash hands during the entire inspection. It was observed that staff performed various tasks without washing hands in between.
      • 2. Twice it was observed that a staff member was making pastas, then came and washed a frying pan at the 3-compartment sink using soap and a sponge, before proceeding to go make pasta again - all without changing gloves and washing hands.
      • Corrective Action(s):
      • - Immediately have all staff wash their hands using liquid soap, paper towels and hot/cold running water at the designated hand washing stations.
      • - Educate and remind staff on the importance, proper method and frequency of hand washing. This includes before starting a new task, after using the washroom, after using chemicals, after touching hair or their face, after coughing and sneezing, etc.
      • - Review glove change policy followed by proper hand washing with all staff. Reminder, gloves do not replace hand washing.

      • Observation:
      • Garbage bag used to cover dough sitting in the dough mixer.
      • Corrective Action(s):
      • Immediately stop this practice as garbage bags are not food grade. Food are only to be covered and protected from contamination using a food grade cover/lid/plastic.

      • Service dishwasher and ensure sanitizer is coming through the pipe and into the machine so dishes are sanitized with 50 ppm chlorine. All dishes in the facility will need to be put through the dishwasher again after it has been serviced and dishes are confirmed to be sanitizing with 50 ppm chlorine.
