- The sink inside the tool reprocessing room was also used for handwashing.- Ensure a written process of Handwashing and Equipment Reprocessing Sink Decontamination is implemented and posted.The sink must be cleaned and disinfected with at minimum a low-level disinfectant before tool reprocessing to avoid cross contamination. - Dirty tools were not soaked in solution when the tools were not reprocessed right the way.- Ensure dirty tools are soaked in solution unless the tools are reprocessed right the way.

- The high level disinfectant solution (HDL5) was reused without confirming the concentration with test strips, and the result was not recorded.- Ensure the manufacturer instruction is followed. Confirm the solution concentration daily when reused and record the result. - Records of the sterility information (lot number and the expiry date) of the pre-purchased invasive sterile equipment kept with client's records were not available for review. Please ensure the sterility information of the pre-purchased sterile equipment used on the client is documented with the client's forms.
- 26. Are cosmetic products appropriate for their intended use and being handled properly?
- Preserved saline was used for reconstituting the botulinum toxin (neuromodulators). Manufacturer's instructions for use specify to use preservative-free saline. Please follow the manufacturer's instructions for use.
