      • 1.Bottles with disinfectants were not properly labeled. Please ensure all bottles holding chemicals are labeled correctly.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • A couch in room 161 was damaged exposing the foam underneath which is not cleanable. Please repair/remove/replace the couch so the surface is smooth, waterproof, and easy to clean
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The disinfectant used to clean toys did not have test strips available. Disinfectants must have test strips available to ensure they reach the proper concentration. Please acquire test strips for the Odoban to test the mixing ratio. If test strips are not available, please switch to Thymox or use another approved disinfectant. Please ensure manufacturer's instructions are followed.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • The bed sheets used for nap time were overhanging and making contact with other children's sheets and cots.Please ensure children's bed sheets are stored individually and separated from each other to prevent contamination.

      • 1. The whiteboards in the classrooms had metal marker trays with sharp edges. Please ensure the edges of these trays are smooth.2. Some plumbing covers were missing and there were holes on the wall in all classrooms. Please ensure the covers are placed to prevent the plumbing/ electrical hazards.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Hand wash sink did not have hand washing supplies. Please ensure hand sinks are equipped with hand washing supplies.
