    • 26. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Critical)?
      • As per the pest control report from March 28: Low levels of droppings and a dead mouse was found behind fridge in Jr toddler room. These were removed and area disinfected with germicide. No droppings were found at the time of inspection.Please do the following:** Continue to contract the services of a professional pest control operator to eliminate the mouse problem and provide all pest control reports to an Executive Officer of Alberta Health Services. ** Remove all mouse droppings and conduct a deep cleaning and disinfection of the facility using a 1000 ppm bleach solution. ** Declutter and organize all areas of the facility.
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • 1) The door sweeps for the front door, junior toddler room exterior door, and pre-1 room door were in disrepair. Please do the following:** Install new door sweeps to help prevent pest entry.2) There were small gaps below the sink in Jr. Toddler and Toddler rooms.** Address all structural deficiencies identified by the pest control technicians.

      • 1. Mice droppings were observed in the various areas of the facility which includes but is not limited to:** Junior Toddler: behind the fridge** Preschool 1: behind the toy cabinet ** Preschool 2: behind the lockersPlease do the following:** Continue to contract the services of a professional pest control operator to eliminate the mouse problem and provide all pest control reports to an Executive Officer of Alberta Health Services. ** Remove all mouse droppings and conduct a deep cleaning and disinfection of the facility using a 1000 ppm bleach solution. ** Declutter and organize all areas of the facility. ** Wash all fabric items and bedding.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Pushpins are currently being used in the toddler classroom. Since they can fall to the floor and pose an injury risk, please replace them with a safer alternative.2. One of the toy cabinets in Preschool 2 is easily moveable. Please ensure it is properly secured.3. The shelf holding the water bottles in the Preschool 1 room is wobbly. Please secure it to prevent tipping.4. Several staples are protruding from the wooden structures in the classrooms. Please secure or remove them to prevent injury.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. One of the ceiling tiles in the junior toddler room is missing. Please install a replacement.2. The foam padding on one of the beams in the toddler room is damaged. Please repair it and ensure the surface is smooth and easy to clean.3. The washroom door of the preschool room is taped. Please remove it to facilitate proper cleaning.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Children were handling potting soil with bare hands during a scooping activity in the preschool classroom. Potting soil can contain added fertilizers, wetting agents, or other chemicals, and soil can easily become trapped under children’s fingernails, increasing the risk of exposure. Please provide documentation confirming that the specific potting soil used is age‑appropriate, non‑toxic, and approved for indoor play with young children.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Play materials such as flour and baking soda were stored in an open tub on a lower shelf in the laundry room. Mouse droppings were observed on the shelf. 2. Raw flour was used to make play dough in the toddler classroom, and children in the preschool classroom were using raw flour as a sensory material. Because raw flour can contain pathogens and is not safe for children to handle, the operator was instructed to discard all raw-flour-based materials.These materials were discarded due to contamination risk. Violation corrected onsite.

      • 1. Mice droppings were observed in the corners of the laundry room.Please do the following:** Continue to contract the services of a professional pest control operator to eliminate the mouse problem and provide all pest control reports to an Executive Officer of Alberta Health Services. ** Remove all mouse droppings and conduct a deep cleaning and disinfection of the facility using a 1000 ppm bleach solution. ** Declutter and organize all areas of the facility. ** Wash all fabric items and bedding.2.       • Three out of four classrooms, including the hallway, are affected by the sewage backup. Sewage water was present on the floor. Carpeted areas in the classrooms are also impacted.The kitchen remains unaffected.Please ensure the following: - Contact a certified restoration company to assess the extent of the damage and conduct professional cleanup.- Provide Alberta Health Services (AHS) with a detailed report from the restoration company outlining the scope of the issue and the remediation efforts undertaken.- Thoroughly clean and disinfect all affected toys, supplies, and surfaces using a 1000 ppm bleach solution.- Wash all fabrics and linens that cannot be disinfected with bleach solution. - Discard any porous materials that cannot be properly cleaned and disinfected.- Structural components such as walls, piping, and HVAC systems may be compromised depending on the severity of the back-up. These should be inspected, cleaned, repaired, and disinfected where possible.- Mould contamination is a potential concern. Any walls, piping, ceilings, and ventilation systems exposed to sewage water must be addressed accordingly.- Remove and dispose of all absorbent materials, including carpets and water-damaged drywall.- Ensure all remaining structural elements are dried as quickly as possible to prevent further damage or mould growth.The facility must remain closed until all remediation work is completed.

      • Preschool Rm 1- Bookshelf by children cubbies is 50.5 inches tall. Tipping concern for safety, ensure it is anchored to wall to prevent tipping. Jan 30/24- Work order already submitted, awaiting for City of Calgary Maintenance to complete work.

      • Junior Toddler room- the disinfectant bottle by the diapering station was unlabelled to identify its content or use.Corrected onsite.
    • 06. Are proper protocols being followed in regards to client care?
      • Room for 1.5-3 yr- the diapering station had some debris on underside. Ensure after diapering is completed, clean and disinfect the diapering pad and underside areas.
