      • **Lots of mouse droppings observed in the kitchen.**A dead mouse was observed in a trap under the sink.

      • Mould was observed on the based of the cabinet in under the kitchen sink. No active leak was observed at the time of the inspection. Please remove damaged materials and replace with new materials that are smooth, cleanable and impervious to moisture.

      • 1) Mop and bucket were being stored in the central kitchen area. Please ensure that the mop and bucket are being stored in a separate area.> Mop and bucket was observed being stored in the kitchen area during the inspection.       • Some dry goods on the counter and in the cupboards were observed to be open. Food is required to be stored in sealed container to protect form contamination.
    • 15. Is the facility free of a pest infestation?
      • An active cockroach infestation was noted throughout the facility, particularly in the following areas: 1) Infant room - cockroaches were observed on sticky traps throughout the infant room in various stages of growth from nymph to adult. Adult cockroaches were observed to be still moving on some of the traps.2) Kitchen - cockroach were observed on sticky traps under the sink and in the cupboards as well as live cockroaches observed at the base of the fridge where the door hinge is located.

      • A dead mouse was observed behind the stove.

      • Children's bottles, cups and other dishes were not being stored in a sanitary manner. Due to pest infestation bottles and other dishes including disposable should be stored in a sanitary and enclosed bin to prevent contamination. Please ensure bottles are dry prior to storage.
