- The mop sink is located directly next to the 2-compartment dishwashing sink and is not enclosed. Please either move the mop sink to a more suitable location or install an enclosure to protect from contamination. Mop sinks should be located at least 10 feet (3 m) away from a food preparation or storage area, unless the mop sink is located in a separate room.

- The sink in the staff washroom was loose from the wall and the caulking was peeling away from the wall. The wall behind the sink was also damaged, had peeling paint, and had a larger hole near the floor. Please repair the wall and re-mount the sink such that it is secure, sealed, and cleanable.

- The staff washroom opens up into the dishwashing area and is not equipped with a self-closing hinge. Please install a self-closing device/hinge such that the door closed after use to limit cross-contamination.

A few mouse droppings were observed under shelving in the back room. please safely clean the droppings and sanitize the affected areas. Jan 9th, 2024: most mouse droppings had been cleaned up. A few droppings were observed under the shelving under the chemical storage. Pest control is being provided by Orkin pest control on a weekly basis. Please safely clean the droppings and sanitize the affected areas. Please continue with the pest control program to abate the pest issue. Mouse droppings were observed on the floor in the staff washroom, beside the Slurpee machines, and on the shelving for chemical storage. Pest control monitoring devices were inspected, and no live/dead pests were observed. Please safely clean up the mouse droppings and sanitize the affected areas. Please continue with the pest control program and provide the undersigned PHI with copies of previous pest control reports.
