    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • - 1. Before and after school room 1: There was no door to the storage room preventing children access. There was a tall shelf ( not secured to the wall), laundry machines, and electrical wires from electronics dangling/ available. ** Please prevent access by either installing a door (with a lock) or gate to prevent children access to these hazards.2. Before and after school room 2: Brown shelf measuring greater than 4 ft high was not secured to the wall. ** Please secure the brown shelf to the wall to prevent it from tipping over. 3. Before and after school room 2: Book shelf measuring greater than 4 ft high was not secured to the wall. ** Please secure the book shelf to the wall to prevent it from tipping over. 4. Before and after school room 2: The door to the kitchen was not lockable and the door is situated near the play area. ** Please ensure a lock is installed for the kitchen door to prevent unauthorized children access to the area. -

      • - 1. Preschool room washroom: The toilet paper roll holder was secured to a raw/ unfinished wooden block to the wall. ** Please apply a proper finish to the wooden block and ensure a smooth, non-absorbent, and easy to clean surface is made (ex. paint). 2. Preschool room washroom: The wall behind the sink was chipped and no longer smooth/ easy to clean.** Please repair the damaged wall and ensure the surface is made smooth, non-absorbent to moisture, and easy to clean. A backsplash may be installed to help prevent future water damage. 3. Ceilings throughout the facility were water stained and some were damaged. This includes but is not limited to: - The preschool washroom- The before and after school care rooms - The before and after school care boys washroom ** Determine the source of water causing for the water staining / damage and properly repair. Then, ensure the area is dried, any possible mould is remediated, and replace all water stained/ damaged building materials with new. ** This work is in progress. The landlord had a company come to assess the damage and the program is awaiting next steps. 4. Preschool room girls washroom: There was a hole in the wall behind the stall door. **Patch the hole in the wall and ensure a proper finish is applied to the area so it is made smooth, non-absorbent to moisture, and easy to clean. -

      • - Before and after school boys washroom: The lights were not operating properly and were flickering. Washroom was very dark. ** Ensure the washroom light is repaired or replaced so the washroom is properly lit. -
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • - Before and after school care boys washroom: One of the toilets were not operational. ** Please repair or replace the toilet and ensure all toilets are properly operational. -

      • Damaged floor observed in the BAS classroom.-Please repair and clean the indicated areas to ensure good repair and sanitary condition.

      • 1. Light fixture without cover was observed in 5-7 years classroom.-Please provide suitable cover for the light fixture or ensure the bulbs are scatter proof.2. Push pins were observed on walls of preschool classroom.-Please remove the push pins as these may fall onto the floor and injure a child. Use tape or sticky tacks instead.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Peeling paint was observed on the walls of the Before and After School (BAS) classroom.-Please repaint the indicated areas to ensure good repair.2. Dirty and damaged floor observed in the BAS classroom.-Please repair and clean the indicated areas to ensure good repair and sanitary condition.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • The paper towel dispenser in the BAS washroom is not in good repair. Paper towels are currently stored in an open container.-Please repair or replace the dispenser. Keeping paper towels in open container may lead to bulk contamination.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid missing tweezers, scissors and blankets.-Please provide the missing items to all classrooms first aid kits.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Garbage bin with damaged foot pedal observed in 5-7 years washroom. The bin' lids was being touched after handwashing, which is unhygienic.-Please repair or replace the garbage bin with one equipped with functional foot pedal.2. Damaged cabinet observed under the hand washing sink in preschool classroom.-Please repair and ensure smooth surface for effective cleaning and sanitizing.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • Ineffective caulking observed around facility's washrooms' toilets and preschool hand sink.-Please apply fresh silicone sealant around the indicated areas to prevent water infiltration.

    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • There is a family of pigeons nesting about the doorway. There is pigeon feces on the sidewalk outside the door and some on the door. Relocate pigeons and seal off the sign above the door where they are nesting.
