      • The following items require cleaning: * The rice cooker requires cleaning. Ensure the tapes used in holding the cooker edges is removed. ** The prep cooler across the cook line is also dirty. Ensure it is cleaned properly. *** The prep cut table is also requires proper cleaning.

Please address the concerns below by December 16, 2025. facilitate sanitary food handling, to prevent entry and harborage of pests. Longer than usual corrective action timeline has been granted to ensure proper and effective correction of outstanding concerns.1. Rubber base boards violation in the dry storage area is still outstanding. 2. The rubber base boards in the side dry storage room were not tightly fitted to the wall.3. Near the entrance to the back area there were multiple damaged tiles on the wall.4. Across from the two-chest freezer the walls have additional board that covers the bottom part. That additional piece is not tightly sealed to the wall and is a potential hiding spot for cockroaches and does not prevent moisture for getting in between the two pieces.

The ceiling in the main storage room had multiple holes. Correct by dec 16, 2025. 3. Ceiling in the food preparation and storage room with upright cooler #4 showed peeling paint and staining, indicating possible water damage. Food preparation in this area is prohibited until ceiling is repaired or replaced. Correct by Dec 16, 2025. Required Action:Ensure all ceiling, wall, and floor surfaces are maintained in good repair to prevent pest harborage and facilitate effective cleaning.

      • Observed multiple wet, used cleaning cloths mounted on a countertop. Corrected during inspection.Action Required.Please store wet, used cleaning cloths in approved sanitizer buckets when not in use, to prevent contamination of food contact surfaces.
    • 07. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Critical)?
      • Observed a dry bulk bean container without a lid. Corrected during inspection.Action Required:Ensure all food containers are covered to reduce the risk of cross contamination.
    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • Observed cooked chicken being held in two plastic trays mounted on countertops, without temperature control. PHI inquired from food handler and was informed that the chicken had been left in the danger zone (20C to 60C for 3-4hrs (more than 2 hrs). Internal temperature of chicken in the two tubs was 13.5C and 14.2 C as measured by a probe thermometer. PHI directed the food handler to adulterate the chicken with concentrated bleach before discarding it in garbage bins at the time of inspection. Action required: 1. Keep cold foods cold ( 4c or colder) and keep hot foods hot (60C or hotter).2. PHI directed Operator to thoroughly clean and sanitize the paper towel holder. Ensure all equipment within the facility is kept in a clean and sanitary manner, particularly hand sink and related accessories to ensure sanitary handwashing procedures.
