- Lower floor washroom has a door that leads to a storage room containing bleach. The handle is loose, and the doorknob cover is not effective.Handle must be fixed or replaced and the door secured.

- Sign advising visitor that there is an outbreak is on the main public door but not the staff entrance.All exterior doors should have the sign posted.

- Sanitizer test papers to check chlorine concentrations were expired in the baby-toddler room. New chorine test papers were obtained from the kitchen.
