    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Contaminated equipment was stored in the sink without a soaking solution to prevent soil from hardening.Corrective Action:- Please ensure contaminated equipment is stored in a manner that prevents soil from hardening. Soak contaminated equipment in an instrument detergent or enzymatic cleaning solution until they are reprocessed.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Equipment in adequate monitoring was not sterile at the point of use.Several peel pouches were not sealed properly along the perforated line as specified by the manufacturer, leaving the packages open with gaps.Corrective Actions:- Ensure peel pouches without adequate sterility assurance are removed from use.- Once all corrective actions have been taken with respect to the sterilization process, repackage and sterilize all peel pouches without previous sterility assurance.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • Sterilization procedures were inadequate:- Load numbers were no longer labeled on sterilization pouches.- Physical parameters were not properly monitored and documented. Sterilization temperature was recorded, however the amount of time the chamber maintained the steriliation time was not monitored or documented.- Type 5 internal chemical indicator was not used for chemical monitoring.- Biological indicator (BI) was last performed in April 2024.- Qualification of the sterilizer was not performed after it was moved from the previous facility location.Corrective Actions Required:- Run a successful BI, type 5 chemical indicator, document sterilization temperature and exposure time.- According to the MIFU, ensure the chamber reaches either 121C for 30 minutes, or 134C for 10 minutes.- Label peel pouches with the appropriate load number. Ensure labels are either on the plastic side of the peel pouch, or if on the paper side, located outside of the seal.
