- 1. Kinder room: A water bottle with glittery water was noted in the room. The bottle cap was not secured or taped shut.The bottle was discarded during the inspection.
- 06. Are proper protocols being followed in regards to client care?
- 1. The diapering procedure was reviewed in the nursery room. The staff member did not change out the gloves after removing the soiled diaper. The staff member then proceeded to dress the child and handle the sanitizer bottle with the same gloves.Education was provided on site. Staff should remove contaminated gloves and use a new set of gloves before dressing the child and sanitizing the space. Hand sanitizer near the diapering station is also recommended.

- 1. Missing items in the first aid kit. The cleaning swabs also were expired in 2020.Please replenish any missing/expired items in the first aid kit.

- Preschool room: Electrical receptacle covers was detaching from wall and was just mended with tape.- The electrical receptacle cover was blocked by a big toy immediately. - Repair the electrical receptacle cover to prevent injury.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- Toddler room: The changing mat had rips on. - Replace the damaged changing mat. Toddler room: Play pillows were stained. - Laundry pillowcases at least weekly or more often if necessary.

- Kindergarten room: Sheets of different children were touching when sleeping mats were stored on top of each other.- Take sheets off sleeping mats after napping and stored them adequately separated.

- Number of used wiping cloths were drying in different classrooms. - Do not handwash wiping cloths. - Used wiping cloths must be adequately laundered and high heat dried before reusing.- Replace wiping cloths after use. - Store clean and used wiping cloths adequately separated, such as in separate containers etc.
