      • Single-use emery boards were visibly contaminated and stored in a drawer. Facility provides both natural and artificial nail services and does not have valid high-level disinfectant. Single-use implements must be discarded after each use.

      • 1) Bottle of high-level disinfectant provided was expired. Please obtain valid disinfectant and cease all high risk personal services including nails and facials.2) Low level disinfectant was not provided for clippers.

      • 1) There was an accumulation of hair in drawers and cabinets. Towels lining drawers were heavily contaminated with hair.
