      • Clippers, combs, brushes, etc. in drawers were contaminated with hair. Please clean items after each use.**Apr 24, 2025: hair present on brushes.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • A cloth runner was on the nail station table. Please remove as skin contact surfaces must be nonporous and easily cleanable.

    • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
      • Single-use emery boards were visibly contaminated and stored in a drawer. Facility provides both natural and artificial nail services and does not have valid high-level disinfectant. Single-use implements must be discarded after each use.
    • 20. Is reusable equipment disinfected properly?
      • 1) Bottle of high-level disinfectant provided was expired. Please obtain valid disinfectant and cease all high risk personal services including nails and facials.2) Low level disinfectant was not provided for clippers.

      • 1) There was an accumulation of hair in drawers and cabinets. Towels lining drawers were heavily contaminated with hair. Please clean and ensure surfaces are easy to clean and maintain.2) Clippers, combs, brushes, etc. in drawers were contaminated with hair. Please clean items after each use.

      • Facials were also provided on the price list. This was a service not advised to AHS previously. Please update AHS on all personal services provided and complete a new Personal Services Notification form at https://ephisahs.microsoftcrmportals.com/create-case-psn/.

      • Paper towels in a dispenser were not provided at the hand wash sink.-Please install paper towels in a dispenser at the hand wash sink.
    • 20. Is reusable equipment disinfected properly?
      • Alcohol was being used to disinfect semi-critical tools.-Please obtain a high-level disinfectant for disinfecting semi-critical tools.
