- A thermometer to monitor sanitizing temp of dishwasher was not provided. Please provide.

- There was a gap under the entrance door. Please eliminate the gap.

- 1) The grease trap lid could not be removed due to the manner of how the wooden support structure around it was constructed. Please make necessary adjustments so grease trap lid can be removed and the grease trap serviced as needed.2) There was a gap along the equipment washing sink and wall. Please silicone.
- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
- There were holes along the concrete perimeter. Please fill in/seal and finish to provide a smooth, nonporous and easily cleanable surface.
