    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Long, dangling wires were observed in the Bunnies 1 and Fantastic Frogs 1 classrooms.-Please secure the wires properly or keep them out of children’s reach to prevent potential entanglement hazards.2. Acrylic paints were observed in several classrooms.-Please remove the bottles of acrylic paint from the classrooms and replace them with non-toxic, washable, and age-appropriate products.

      • Torn diaper mats were observed in the Baby Bunnies 1 and Fantastic Frogs 2 classrooms.-Please replace the identified mats with new ones and ensure the surfaces are smooth, non-absorbent, and easy to clean.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • A vent in the Fantastic Frogs 2 classroom was observed with a noticeable layer of dust.-Please clean the identified vent and ensure it is maintained in a sanitary condition at all times.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Sanitizer concentration (bleach solution) was inadequate, found to be 50 ppm.-Please adjust the solution to ensure the sanitizer concentration reaches 100 ppm.

      • In current 4+ room, cardboard egg carton was being used for an activity, and some paints labeled as "keep out of reach of children" found.-Ensure staff review all toys used for play, and arts and crafts, and exclude items that are unsafe-Operator removed the items
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1) New side BAS bathroom - sink cabinet missing baseboard, exposing an absorbent surface.-Seal the wood or add a baseboard to the cabinet base prevent moisture damage2) New side BAS shelf above coat hooks - sides of the planks have an exposed absorbent surface.-Seal the surface to prevent moisture damage
