      • -Gold streamers were being used for sensory in Room 2. They were left out and not covered when not in use. Streamers could be a strangulation hazard. Please do not use.-Foam shapes in Room 4 had bite marks and pieces missing; this is a potential choking hazard. These shapes are not recommended for younger children. If they are going to be used please remove damaged pieces.

      • -Staff member in Room 3 was observed changing diapers and did not wash children’s hands or their own hands. Ensure that staff changing diapers wash their hands when changing gloves and wash children’s hands.-Ensure diaper procedures are posted in Room 3 and Room 1.

      • -Cleaning checklists not being filled out in Room 1 and Room 3. Please ensure that checklists are being filled out.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • -The Little Tykes kitchen in Room 5 was unsteady and coming apart. Please repair/replace.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • -The top cot in Room 5 was turned upside down so the bedding was touching the cot below. Please ensure that cots are stacked so that bedding does not touch.

      • -The potty seat in the washroom for Room 5 was stored on the washroom floor. Please store up off of the floor on a hook or in a container.

      • Several walls noted to have chipped paint. It appears that renovation has been started to refresh the paint. Please ensure all walls are in good repair and easily cleanable.Cupboard door within children reach (room 3) was noted to be in disrepair. Screw became undone. This cupboard was noted to not be locked at the time of the visit. Ensure all equipment is in good repair.

      • Several walls noted to have chipped paint. It appears that renovation has been started to refresh the paint.
