- -The diapers mats in the Toddler Room and Young Preschool Room were torn and split. Have these replaced to allow for proper cleaning and disinfecting.

- -There were cracks/wear/peeling paint on the walls around the handwash sinks in the following areas:-The Baby Room-Young Preschool Room-The boys and girls washrooms in the OSCHave these areas repaired to prevent further water damage.-A CD player was placed right next to the handwash sink in the baby room. Move this to another area to prevent obstructing the sink; knocking it over; and causing a shock hazard.

- First-aid kit was inadequately supplied. Alcohol wipes were found expired (2012) and were removed onsite. Obtain an appropriate first-aid kit.

- 1. The temperature of hot water in the washroom was measured at over 54°C in the Preschool-KG room (4-5 years). Adjust the temperature of the hot water to a maximum of 54 °C at fixtures accessible to children.2. The water pressure in one of the sinks in the toddler room (19 months to 3 years) washroom was low. Adjust the water pressure to ensure sufficient flow at the sink.

- -There were large bottles of hand soap under the sink in the Lotus Room; these are considered a chemical. They were moved out of the reach of children at the time of inspection.-There was a purse with long straps in the Lotus Room; this is a potential strangulation hazard. It was removed at the time of inspection.

- -There were two water stained ceiling tiles in the Daisy Room. Operator indicated that the source of the leak had been repaired. Please ensure the tiles are replaced.
