- Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms. Discussed.

- Operator was unable to notify the Alberta Health Services of existing personal service business.Personal Service Notification was submitted during the inspection, as directed by the PHI.

- Written procedures, applicable to the business were discussed with the operator.Based on the services provided at the facility, following procedures were required.1)Written procedures for accidental cuts and punctures.2)Written procedures for skin cleansing.Templates for written procedures were provided during the inspection.Please note that these templates are for reference only, facility can use these or modify it as required.
- 20. Is reusable equipment disinfected properly?
- During the inspection it was observed that the operator was using the High-Level Disinfectant Pre-empt CS-20 for disinfecting the semi-critical tools.Following observations were made;1)Operator was diluting the HLD with the water.2)HLD was being reused for almost a week, without testing and maintaining the logs.Please make sure to follow manufacturer's instructions for use of HLD.HLD can be reused for 14 days, daily testing and log maintenance is required (Based on the choice of disinfectant AHP7 are the recommended test strips).Logs would be provided with this e-mail.
