Mitigate pest infestation and develop and implement a proper pest control plan.A copy of a pest control checklist will be provided via email.

- The facility was found to be in overall poor sanitary condition.Accumulation of dirt, grease and food debris was observed throughout the kitchen (dirty floors, ceiling tiles, air vents, exterior of equipment, inside coolers on shelves etc and grime on food containers, under coolers, shelving and other equipment). Walls were found to be damaged and in poor condition making them not easy to clean. The inside of the coolers are in need of cleaning.See Closure Order A copy of a cleaning schedule and sanitation program will be provided via email.
