      • Plush toys, uncleanable play equipment such as cardboard and fabric items were seen in some classrooms. Until the outbreak is over, please remove the items that cannot be cleaned and disinfected properly.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • 1. Room 1: Used cleaning rag was hung on the spray bottle. Please ensure the following:- cleaning rags are used only once and then disposed of or laundered. This prevents cross-contamination and bacterial spread. - place clearly labeled bins in each classroom for used rags.2. Room 3: The concentration of the bleach solution was 500 ppm. The operator prepared fresh solution tested 1000 ppm.

      • 1.The storage cubbies in room 2, 4, and 3 on the walls by the entrance were not properly secured or anchored, creating a fall risk for children. Please anchor the storage cubbies to the wall so they are secure and cannot fall over.

      • 1.The storage cubbies in room 2, 4, and 3 on the walls by the entrance were not properly secured or anchored, creating a fall risk for children. Please anchor the storage cubbies to the wall so they are secure and cannot fall over.2. Room 4 had lego pieces out for use creating a choking hazard for children under the age of 3.Please remove the lego from room 4 and only use in areas where children aged 3 and above are present.

      • Craft items that were not appropriate for the age of users were found in rooms 2, 4 and 3. Items including but were not limited to beads, shaving cream, conditioner, glue, and hydrogen peroxide. Items were removed from the facility or stored out of reach of the clients. Please look through all old shelving and discard of any items that are labeled unsafe for children or do not have a child safety certification (ACMI). If unable to look through all shelving, please put lockable doors on the shelves instead, or remove the shelves entirely.

      • Operator was using QUAT as a disinfectant instead of bleach or accelerated hydrogen peroxide. QUAT is not an effective disinfectant for diapering stations.Operator obtained a bottle of bleach to replace the QUAT at each diapering station.

      • Please clean the following areas- the diapering area in room 2 underneath the mat

      • 1) Paint chipping and exposed drywall in Room 3 and Room 5 washrooms.- Refinish the surface and ensure surface is smooth, non-absorbent, and easily cleanable. 2) Rusting of heater unit next to toilet in room 3 upstairs.- Refinish the surface and ensure surface is smooth, non-absorbent, and easily cleanable. 3) Corner of wall leading to basement washrooms is in disrepair.- Repair the wall and ensure surface is smooth, non-absorbent, and easily cleanable.
