    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Several electrical outlets in the classroom missing outlet covers. Place covers for all outlets.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1. Chlorine spray bottles were stored in the back room and was not available for use during operation.2. No chlorine solution was used for toy cleaning and dishwashing.Ensure that chlorine solution at 100ppm is prepared for dishwashing, toy cleaning and surface cleaning. Staff prepared a chlorine solution at 100ppm during inspection.

      • Wooden toys observed with string materials wrapped around it. The wood material has coated surfacing. Remove strings and ensure that all toys are smooth, non-porous and easy to clean.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • No adequate storage is available to store cleaning and dishwashing chemicals near the two-compartment sink. Ensure that a storage unit is available by the toy-cleaning and dishwashing sink area for each access to required sanitizer and detergents. Ensure that the chemical storage unit has childproof locking to restrict children's access.

      • Wicker baskets were observed to be used to store toys. This material is not considered easy to clean and disinfect and could cross-contaminate cleaned toys.Use a liner material inside baskets that can be cleaned or disposed after use to protect stored toys and baskets from potential contamination.

      • The flooring in the Nest room was needs to be adjusted to avoid any trips and fall.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • PUREXALL disinfectant was expired.Please make sure there is new disinfectant solution is available for sanitation of the food contact surfaces /tables, and high touch surfaces. Also test strips need to be available onsite to measure the concentration of the solution.

      • Container used to store toys are dusty and need cleaning, benches need cover to protect and easily clean them as the surface is coming off. Water fountain taps appear dusty and need to be clean with bleach water solution.

      • Oxygen mask toy must be disinfected between children to prevent the spread of any illness between children. Bleach water solutions recommended as a disinfectant to use as a toy cleaning solution.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • Temperature at the washbasin was measled as 53.5C, asked the operator to reduce it and must not exceed more than 49C. Until the temperature is corrected, one of the staff to help children while washing their hands to protect their hands from scalding.
