      • Observation: 10ppm chlorine detected from the low temperature dishwasher after operator switched to a new container of sanitizer.
      • Corrective Action(s): There must be 50-100ppm chlorine residual for sanitizing. Call technician immediately.
      • Facility will be closed tomorrow. Staff are directed to wash all dishes in the dishwasher as usual and manually sanitize at the 2 compartment sink until the dishwasher is serviced and has at least 50ppm chlorine residual.

      • Observation: Rodent droppings were observed in the following areas:
      • 1) Under the dishwashing line and chemical storage shelf
      • 2) Behind the ice machine
      • 3) Under the light switches across from the handsink
      • 4) In the space between the keg cooler and counter
      • 5) Under the keg cooler and reach in freezer
      • 6) On the storage shelf across from the keg cooler - shelves 3 and 5
      • 7) On and around the equipment storage shelf by the window - bottom shelf
      • 8) Under the prep table in front of the 6 burner stovetop
      • 9) In the space behind the walk-in cooler in front of the back exit
      • Corrective Action(s): Remove all droppings, clean and sanitize the floor in these areas with bleach.
      • Pull all stored items away from the walls and off the shelves for proper access to clean.
      • Date to be corrected by: November 26, 2025.

      • Observation: Several potential entry points noted today:
      • > Gaps in the ceiling panels above the walk-in cooler
      • > Gaps in the ceiling where there is access for electical (one above the keg cooler, another one above the light switches across from the handsink)
      • > Gaps in the ceiling that were previously used for electrical access (one above the entry way from dining room to kitchen)
      • Significant clutter noted in the following areas:
      • > The space between the counter and keg cooler
      • > The space behind the walk-in cooler and in front of the back exit
      • Corrective Action(s): Discussed with operator:
      • 1) Seal ALL potential entry points. The gaps above the ceiling were mentioned two routine inspections ago. They must be sealed now.
      • 2) De-clutter the kitchen. Rodents are likely to hide in spaces that are cluttered and hard to see. Items that can be put on a shelf must be stored on a shelf. There must be minimal items that are stored directly on the ground or stacked on top of each other.

      • Observation: Low temperature dishwasher is unable to drain automatically after each cycle. Sanitizer entering the machine is being diluted by the residual soap/detergent.
      • Corrective Action(s): Call a technician immediately. It is not practical to have to manually drain the dishwasher after each cycle.

      • Observation (CORRECTED DURING INSPECTION): All spray bottles containing bleach solution measured below 100ppm chlorine. No chlorine test strips were available on site.
      • Corrective Action(s): Refill spray bottles so that the contents measured 100-200ppm chlorine. The solution must be remade daily at the start of each day.
      • Obtain chlorine test strips.
      • Violation Score: 5
    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Droppings were observed in the following areas:
      • > Under the handsink on the service line
      • > Bottom shelf of the undercounter storage space on the service line
      • > Under and behind both dry storage racks near the West side of the kitchen
      • > By the back exit door
      • > Around the grease trap
      • > Under the 2 compartment prep sink
      • Corrective Action(s): Remove droppings by pulling all stored items off the shelves and pulling the shelves away from the walls. Clean and sanitize these areas with a bleach solution. Inspect for new entry points and seal immediately. Continue to monitor for rodent activity.

      • Observation: Several holes and large gaps noted in the ceiling panels in the kitchen. These can be potential rodent entry points.
      • Corrective Action(s): Seal holes, minimize the size of openings around electrical wires or pipes, and replace ceiling panels as necessary.

      • Observation: General cleaning for food debris and grease accumulation is required under the cook line, under the dishwashing line, and on the walls/pipes nearby the back exit
      • Corrective Action(s): Remove food debris and grease accumulation - these can be food sources for rodents.

    • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): Low temperature dishwasher had 0ppm chlorine residual for several runs at the start of the inspection. Sanitizer container was noted to be half-full, and sanitizer line was primed, however there was still 0ppm chlorine.
      • Corrective Action(s): Staff changed the sanitizer to a new container and primed again. 50ppm chlorine residual recorded.
      • Recommend to have the dishwasher serviced. Regular maintenance must be scheduled.

    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Significant amount of rodent droppings were observed in the following areas:
      • > Under the entire dishwashing area
      • > Behind the ice machine
      • > Behind the dry storage rack
      • Corrective Action(s): Remove droppings. Clean and sanitize these areas with a strong bleach solution to help remove rodent urine and scents.
      • Date to be corrected by: January 3, 2025

      • 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
      • Observation: There is clutter in the kitchen mainly in the front half where there is dry storage. Large equipment, stacked boxes, and personal belongings are stored on the floor. Large equipment are blocking rodent entry points and concealing signs of activity (eg. chewed insulation material).
      • Corrective Action(s): Declutter the kitchen. All items must be stored at least 15cm above the ground to help with cleaning and to minimize clutter that attracts rodents. Obtain additional shelving if necessary. Do not over-order equipment or supplies.
      • Date to be corrected by: January 3, 2025.

      • Observation: Cleaning is required in the following areas:
      • > Under the cook line - food debris
      • > Parts under and around the vent hood (pipes, other exterior surfaces) - grease accumulation
      • > Prep line - food debris under the coolers
      • > On top of the dishwasher - food debris
      • > On top of the walk-in cooler - general debris, cardboard liners
      • Corrective Action(s): Clean and sanitize the premises as per your sanitation plan. Maintaining a sanitary environment will prevent the presence of pests.
      • Date to be corrected by: general cleaning must be completed by January 3, 2025. Staff are to deep clean the facility by the end of the month.

      • 307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
      • Observation: Walk-in cooler door gasket is in poor condition. Staff states it has been in this condition ever since taking over ownership 2 years ago and replacement parts are not available.
      • Corrective Action(s): Old duct tape and electrical tape used on this part of the gasket is also in poor condition and no longer sticking. Replace the tape with a more suitable material that can is non-porous and can withstand cleaning.
      • Date to be corrected by: January 3, 2025

      • Observation: Wall baseboard under the 2 compartment sink next to the stove required re-patching. There is exposed wall and clear signs of rodent activity here.
      • Corrective Action(s): Repatch the wall ASAP. This area must be pest proofed.

    • 402 - Employee does not wash hands properly or at adequate frequency [s. 21(3)]
      • Observation (CORRECTED DURING INSPECTION): Staff was observed to be using their cell phones while working in the kitchen. No hand washing was observed.
      • Corrective Action(s): Staff was asked to wash their hands at the time of inspection. Do not use personal items while handling food. Hands must be washed every time you change activities - especially after handling potentially contaminated items (personal items, chemicals, floor, garbage, raw meats, raw eggs, etc.).
      • Violation Score: 5
    • 209 - Food not protected from contamination [s. 12(a)]
      • Observation (CORRECTED DURING INSPECTION): 1) three containers of chopped lettuce, carmalized onions, and a cream-based sauce were kept uncovered in the under counter cooler on the bottom shelf.
      • 2) A bag of raw meat was kept above cooked soup/broth in the walk in cooler.
      • Corrective Action(s): 1) The containers were covered with lids at the time of inspection. Ensure food is stored in containers with tight fitting lids to prevent cross contamination.
      • 2) Raw meats, raw eggs, should be stored on the bottom most shelf to prevent cross contamination of ready to eat foods.
