      • Mushroom gravy had a printed sticker that indicated when it was created and when it was to be thrown out, 2 hours after it the food was made. During the inspection, it was found that the gravy was to be thrown out nearly 3 hours ago. This was brought to the attention of the nearest staff member, and the gravy was promptly discarded and the gravy dispensing unit was replaced.Please ensure that all food items held at room temperature are discarded at appropriate times as indicated by their label.

      • Gravy was observed being handled and served by employee beside the hand washing sink. Rice was also observed being handled in the same area. Please ensure high risk foods are handled and stored away from the handsink to prevent splashing and contamination.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • One of the handsink's soap dispenser handle was broken and not working. Please ensure the handsinks has a working soap dispenser.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • The posted food handling permit was not visible to the public. Please ensure the food handling permit is posted in a location that is visible to the public.
