    • 06. Are proper protocols being followed in regards to client care?
      • Staff were observed changing diapers and then using dirty gloved hands to handle a pen, and one was observed not washing hands after removing gloves. Hand washing was observed at a sink where food had been set in close proximity. Food can be contaminated by water when washing hands after diapering. Staff was advised to discard food in near vicinity. Items not associated with diapering should be handled with clean hands and hands must be washed before and after glove use. Ensure snacks/meals are placed away from the hand washing sink at all times. Diapering table was in good condition with a secure belt, gloves and disinfectant were used during procedure. Ensure the table is cleaned beneath the change mat as well.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • There is a slide that is aprox 2 feet high at the top of the slide with only a narrow mat beneath it that doesn't extend out the sides of the slide. Please ensure there is proper fall protection on all sides of the slide and the slide is not situated close to a wall that children can fall into if the slide should tip. There should be 6 ft of unobstructed area around the slide that has properly rated fall protection. Fall protection should be rated for a fall height at least as high as the top of the slide platform.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • One staff observed not washing hands after changing a diaper and then removing gloves. Hands can be contaminated when removing dirty gloves, and gloves can be contaminated when putting them on, if hands are not washed before glove use. Please ensure all staff are trained to wash hands before and after glove use.
