      • Procedures still not completed. Operator completed the skin antiseptic and has the accidental exposures on site. Please provide a copy of the skin cleansing and after care. August 18, 2022The operator demonstrated their procedures during the inspection. Written procedures for preparation of skin were not completed. Please fill out the written procedure and send a copy back to the district PHI.

      • Client records observed did not include description of personal service and the name of the personal services worker who provided the personal service. Please ensure both of the above are added to the client records form.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Operator indicated that distilled water is used for mixing with inks. Please ensure sterile water is used for mixing inks and immediately discontinue use of distilled water for mixing in inks.
